LONG TERM CARE DIVISION

Passionate Long Term Care is a division of Passionate Health Care Services, Inc., a Medicare and Medicaid certified company committed to providing quality healthcare at home. We offer multiple programs, covering the entire scope of home health needs as well as home making services. These services are provided under the licensed category of home health service.

PRIMARY HOME CARE (PHC)

Primary Home Care is a medically related personal care service prescribed by a physician as part of a client’s plan of care. Primary Home Care is available to eligible Medicaid clients whose health problems cause them to be functionally limited in performing activities of daily living. Each client’s services are provided by a Primary Home Care attendant.
Primary Home Care assists the client with:

  • Housekeeping
  • Laundry
  • Meal Preparation
  • Grocery Shopping
  • Personal Care
  • Escort to Physician’s office

FAMILY CARE

Family Care is a service that is provided to eligible aged or disabled adults who are functionally limited in performing daily activities. This program is block grant funded. Services may be provided by a person able to have a family-like relationship with the client.
Family Care assists the client with:

  • Housekeeping
  • Laundry
  • Meal Preparation
  • Grocery Shopping
  • Personal Care
  • Escort to Physician’s office

COMMUNITY BASED ALTERNATIVE (CBA)

Community Based Alternative is a program which provides home and community based services to person aged 21 and older who qualify for nursing facility care. It is an alternative to nursing home placement, and it allows current nursing facility residents an opportunity to return to the community.
Community Based Alternative assists the client with:

  • Housekeeping
  • Laundry
  • Meal Preparation
  • Grocery Shopping
  • Personal Care
  • Escort to Physician’s office
  • Monitoring by a Skilled Nurse
  • Medical Equipment Purchase
  • Minor Home Modification for the Handicapped
  • Physical and Occupational Therapists if necessary

How do I pay for Home Care?

Passionate Healthcare Services accepts most major insurances, including Medicare and Medicaid.  In addition, Self-pay is an alternative option for our patients and their families.  Please don’t hesitate to contact our office.  Our teams of professional are standing by to assist you with any questions or concerns….

There are usually several criteria required for Medicare to pay for your home health care services:

  • You must be under the care of a physician
  • You must have a limited ability to leave home
  • Care is provided on an intermittent basis
